What is the boarding medical certificate?

The ISM medical certificate is one of those procedures that seems straightforward but can leave you stranded if you don't manage it properly. I've seen colleagues miss embarkations because they couldn't get an appointment in time.

The boarding medical certificate is a mandatory document for anyone wanting to work professionally aboard a vessel in Spain. It's issued by the Instituto Social de la Marina (ISM), a body under the Ministry of Inclusion, Social Security and Migration.

Without this certificate you cannot hold any professional position on board, from skipper to deckhand, chef or stewardess. Along with nautical certifications, it's the most important document for working at sea.

What tests are included?

The boarding medical examination includes:

  • General physical examination
  • Blood pressure check
  • Blood and urine analysis
  • Vision and hearing tests
  • Electrocardiogram (depending on age and position)
  • Basic psychological assessment
  • Medical history review

The goal is to verify that the worker is in adequate physical and psychological condition to perform duties on board, including emergency situations.

Where to get it?

The certificate is obtained at ISM centres across the Spanish coast. The main centres in the Balearics are:

  • Palma de Mallorca — ISM Provincial Office
  • Ibiza — ISM Local Office (Calle del Puerto)
  • Mahón (Menorca) — ISM Office

There are also centres in all major mainland ports: Barcelona, Valencia, Alicante, Málaga, Cádiz, Vigo, etc.

How much does it cost?

The ISM medical examination is free for maritime workers registered under the Special Maritime Regime. For those applying for the first time without being registered, the cost may vary, but it's generally free or has a minimal cost.

Validity and renewal

  • Under 50 years old: valid for 2 years
  • Over 50 years old: valid for 1 year
  • Specific positions (diving, passenger transport): may require more frequent examinations

Practical tips

  • Book your appointment in advance, especially between March and May when demand is high
